EuroPACS 2004, nya paradigmer och ny ekonomi

I sitt inledningsanförande pekar Osman Ratlib från USA, på några viktiga aspekter för att bygga hållbara PACS system, nämligen öppen källkod, säker öppen trådlös kommunikation och flexibla arbetsflöden:

"The evolution of PACS is also influenced by emerging disruptive technologies such as wireless communication and open source software components. Accessing images anywhere-anytime has become a reality with secure wireless communication tools. This has changed significantly the basic architecture of image archive and communication systems to adapt to higher on-demand workflow.The rapid adoption of open source software which are often more robust and better designed than commercial products also raises a whole new set of questions and options. The continuous battle between user's demand and vendors-driven solutions is accelerating with the wider adoption ofPACS and higher expectation on accessibility of medical images in clinical practice. PACS technology is evolving from monolithic complex systems to distributed commodity components relying on underlying generic IT infrastructure."

Några andra talare addreserade kostnadseffektivisering och integration i öppna löst kopplade system. T.ex.Fabio Fioravanti visar hur man bygga billiga och öppna PACS system och adressera problemet och integrera det med övrig dokumenthantering. Jarmo Reponen pratade om införandet av mobila "tablets" och menar att största effekten nås om man kan integrera PACS och EMR (Electronic Medical Record).

Under temat integration - "Integrating the Healthcare Enterprise" - fanns flera sessioner om HL7, DICOM och Workflow i olika aspekter. Inledningstalaren Claudio Saccavini sa:

"IHE is a technical consortium under the Healthcare Information and Management Systems Societythat is building a technical framework to encourage the integration of health care systems. Ratherthan creating new standards to define this framework, the group relies on existing standards, one ofthem being ebXML On 17 June 2004, IHE issued for public comment four draft technicalsupplements to its Infrastructure Technical Framework, with one devoted to Cross-EnterpriseClinical Documents Sharing, or XDS. The XDS document specifies the ebXML registry (version2.0) as a key part of its proposed solution. Is this the solution to realize the EHR?"

Två initiativ som är på väg att bli (eller redan är) öppen källkod är den java-baserade DPACS-2004 och DICOM workflow-systemet MARIS som är baserat på programspråket PHP, MySQL och PACS biblioteket CDT och stödjer flera IHE arbetsflöden och aktörer. MARIS är en fortsättning på ett tidigare projekt som hette Raynux. En intressat kommentar om vikten av arbetsflöden från MARIS presentationen:

"The knowledge of IHE actors is very important to design the new radiologic informatic system because the new systems must be workflow based and not application oriented."

Beträffande sessioner om bildbehandling ser man en klar trend i behovet av Grid-teknologier. I USA sponsrar NSF ”National Middleware Initiative” som också har fokus på Grid computing och befrämjar både användandet och utveckling (tätt samarbete med Internet2 Middleware). Två exempel från NMI är Digitala ID och GRID Software,det sistnämnda har resulterat i grid ramverk med öppen källkod t.ex. Globus och Condor.
